"Fatty liver" shows up in a lot of routine bloodwork these days — an ultrasound finding, a slightly elevated ALT on an annual physical, a doctor's note to "watch it." Non-alcoholic fatty liver disease (NAFLD) now affects something like a quarter of adults worldwide, which is probably why "does hydrogen water help with fatty liver" gets typed into Google more than you'd expect. Most of what comes back is generic antioxidant enthusiasm. What doesn't come back very often is what the actual human trials measured, or how honestly to read them.

So that's this post: not a claim that hydrogen water fixes a liver diagnosis — nobody has shown that, and we're not going to imply it — but a walk through exactly what researchers found when they gave real people hydrogen-rich water and checked their livers afterward.

Why the Liver, Specifically

The liver processes almost everything you eat, drink, or take, which also makes it one of the more oxidative-stress-exposed organs in the body. Molecular hydrogen (H2) has been studied for its ability to selectively neutralize the most damaging reactive oxygen species — hydroxyl radicals and peroxynitrite, specifically — while leaving the reactive oxygen species your cells actually use for signaling alone. It's also small enough to cross into mitochondria freely, and it activates Nrf2, a pathway that switches on the body's own antioxidant enzymes.

None of that is liver-specific by itself. It's just the reason a handful of research groups have pointed hydrogen water at liver fat directly, instead of lumping it into general "wellness" outcomes.

The Pilot Study: 12 People, a Liter a Day, 28 Days

In 2019, a team led by Darinka Korovljev — with Tyler LeBaron, a name that turns up across a lot of this field's research, as a co-author — ran a small crossover trial in overweight adults already diagnosed with NAFLD (Korovljev et al., Clinics and Research in Hepatology and Gastroenterology, 2019). Twelve people, average age mid-50s, drank about a liter of hydrogen-rich water a day for 28 days, then switched to plain water for 28 days, or the reverse order — that's what "crossover" means, everyone acts as their own control.

The researchers didn't rely on blood work alone. They used dual-echo MRI to image liver fat directly, before and after each phase. During the hydrogen-water phase, average liver fat signal dropped from about 284 to about 257 on their MRI scale — roughly a 10% reduction — with no comparable change during the placebo phase. AST, one of the standard liver enzymes on any metabolic panel, also fell by about 10%.

Twelve people is a pilot study, not a verdict. But an actual imaging endpoint, rather than a symptom questionnaire, is unusual enough in this research area to take seriously as a starting point.

The Bigger Trial Didn't Reach Significance — Worth Explaining, Not Hiding

Three years later, a Slovak research group (Kura, Szantova, LeBaron, and colleagues, published in Antioxidants, 2022) tried to build on it: 30 people with NAFLD, randomized, double-blinded, placebo-controlled, for eight weeks — longer, and more than double the sample size.

The results moved in the same direction as the pilot study: about a kilogram of average weight loss, a favorable trend in lipid profile, lower lactate dehydrogenase, decreased inflammatory markers (NF-κB, HSP70, MMP-9). None of it reached statistical significance (p > 0.05). Liver enzymes didn't move meaningfully either way. Two oxidative-stress markers even ticked up slightly, which the authors floated as a possible hormetic effect — the same "small stressor, bigger adaptive response" pattern seen with exercise — that may resolve in longer-term studies.

We're not going to dress up a null result as a positive one. It isn't. But a trend in the same direction, in a longer and larger study, is meaningfully different from one small study standing alone — even when the p-value doesn't cooperate. The paper's own conclusion says as much: promising, not proven, more research warranted.

A Third Study Asked Why, Not Just What

A follow-up from the same Slovak group (Sumbalova et al., International Journal of Molecular Sciences, 2023) went looking for a mechanism instead of an outcome. In NAFLD patients given hydrogen water as an add-on therapy, they measured rising coenzyme Q10 levels and improved mitochondrial bioenergetics in blood platelets — a proxy researchers sometimes use for what's likely happening in cells that are harder to biopsy, like liver cells. It's a smaller, more technical piece of the puzzle, but it lines up with the broader mechanism: hydrogen's effects seem to run through mitochondrial and antioxidant pathways, not a single drug-like target.

Zooming Out: A 2024 Meta-Analysis of 433 People

The most recent word on this, as of this writing, is a systematic review and meta-analysis published in December 2024 (Khalili-Tanha et al., Gastroenterology and Hepatology from Bed to Bench), pooling eight randomized controlled trials — 433 participants total, spanning NAFLD, chronic hepatitis B, and cancer patients undergoing treatments known to stress the liver. Across that pooled data, hydrogen-rich water was associated with a slight decrease in ALT, AST, and ALP — the three enzymes on any standard liver panel. The authors' own words: hydrogen water "may decrease liver enzyme levels in clinical populations," and "further research is needed to confirm this relationship."

That's the accurate way to say it, so that's the way we're saying it.

What This Doesn't Mean

None of these studies were done in healthy people trying to stay healthy — they were done in people who already had diagnosed liver conditions, mostly NAFLD. None of them used a True Marqet bottle specifically, and no hydrogen water product, including ours, is intended to diagnose, treat, cure, or prevent any disease — that's not a legal line we're tacking on, it's just accurate. If you have elevated liver enzymes or a NAFLD diagnosis, the things with real consensus behind them are still diet, weight management, alcohol reduction, and your doctor's guidance. Hydrogen water sits, at best, in "worth discussing as an adjunct" territory, not a replacement for any of that.

None of the trials specified an exact concentration threshold beyond "high-concentration" HRW, generally read as above 1 ppm dissolved hydrogen — worth knowing if you're comparing bottles, since tested PPB output varies a lot across the category and isn't always disclosed.
